*RN Case Manager Home Health* We are currently looking for an RN to cover the *Guthrie and Stillwater areas*.
Home office is in Guthrie with coverage area 45 mile radius.
At times you may need to see a patient in another area.
*RN Case Manager Home Health* *Job description* The *RN* *Case Manager* is responsible for providing nursing expertise and ensuring all services needed are identified, outlined in the Plan of Care and delivered to hospice patients and families.
The RN Case Manager is assigned visits, admissions and RN triage duties.
The RN Case Manager is a member of the Interdisciplinary Team (IDG).
The RN Case Manager instructs client and family on techniques of patient care and completes all required documentation complying with the state and federal regulatory requirements.
*RN Case Manager Home Health* *Requirements:* * Current OK RN License * One year of home health or hospice experience is helpful * Complies with acceptable professional standards and practice * Customer-service oriented attitude * Demonstrates good communication, negotiation and public relations skills Salary is dependent upon experience and knowledge, $65,000-$72,000.
Home office is in Guthrie, OK and coverage areas are 45 mile radius.
